Eligibility and Address Q & A
Yes. Some Open Card members can have some of their benefits with Open Card while also having some of their benefits with a CCO. This does not apply to all Open Card members.
People can be partly enrolled in either Open Card or a CCO for their:
- Physical health care.
- Behavioral health, which includes both mental health care and substance use treatment.
- Dental health care.
- Non-emergency medical transportation (NEMT)
For example, an Open Card member can have Open card for Physical and Behavioral Health care, and a CCO for their Dental care.
If you are enrolled with a CCO for any of the above benefits, you need to use the CCO’s contracted health care providers for those health care services. This is also called using an in-network provider. For those services you will need to contact the CCO to have them help find a provider for the type of care. OHP Open Card will help you find in-network providers for benefits that you have covered by Open Card.
If you are enrolled in a CCO for any benefit, you will also use their NEMT benefit services for all your transportation needs.

- American Indian/Alaska Native Open Card members can call CareOregon Tribal Care Coordination at (844) 847-9320.
- If you are a member of a CCO and need help using benefits, please contact your CCO instead.
- Call OHP Client Services at (800) 273-0557 if you do not know if you have CCO or Open Card, want to ask to switch between CCOs or Open Card, or get a bill
- Call the Oregon Department of Human Services’ ONE Customer Service (800) 699-9075 for questions about qualifying for OHP, renewals, or to report changes
All new OHP members start on OHP Open Card for a few days after signing up. Most OHP members will be enrolled in a CCO after 2 or 3 business days.
Some people can or will remain on OHP Open Card:
- People with other Health Insurance: If you have other health insurance such as an employer plan in addition to OHP, you will remain on OHP Open Card. When you get care, show both your insurance card and your OHP ID card..
- People with Medicare: If you have Medicare, you can choose between OHP Open Card and a CCO for your physical health care. You will be enrolled in a CCO for your behavioral and dental health care.
- American Indians/Alaska Natives: If you are American Indian/Alaska Native, you will remain on OHP Open Card unless you ask to enroll in a CCO. You can make this choice at any time.
- If you need to continue care out of network: If you have a serious health need to continue care with a health care provider who accepts OHP Open Card, but not your CCO, you may be able to request temporary open card. You will need to work with your providers to make this request.

OHP members also receive a OHP coverage letter that will tell you whether you are enrolled in a CCO. This usually arrives a week or so after signing up for OHP, or any time your plan changes.
For most members the best way to find OHP Open Card providers is to call OHP Open Card at 800-562-4620.

OHP covers rides to care. OHP Open Card members may need to call a different ride service than CCO members for their rides to care. To find contact information for ride services, visit OHP.Oregon.gov/Rides
Yes! OHP Open Card covers all the same benefits as OHP, including medical care, mental health, basic dental care, and prescriptions.
There is only one difference in benefits, known as “flexible services.” Unlike CCOs, OHP Open Card does not have the option to pay for flexible services that are not normally covered by OHP. For more information about flexible services, visit OHA’s flexible services page, or read this blog post for examples.
However, OHP Open Card may be able to cover many of those same services for members who have certain risk factors through Health Related Social Needs.
OHP Open Card may be able to help you with needs such as housing instability, housing quality, food insecurity, employment, personal safety, lack of transportation and affordable utilities, and more.

To get started using care, please contact OHP Open Card at 800-562-4620 to find health care providers.
New members will receive a welcome packet and OHP Open Card handbook in the main about a month after they sign up for OHP. You can also request another copy at any time or get a copy in an alternate format such as large print or other languages.
If they do not already have one, new members should schedule a check up with a local primary health care provider (your regular doctor) and a dental check up or cleaning. Even if you are not sick, these are important health care services that help you stay healthy. OHP Open Card members will not have a health care provider assigned to reach out to them. To get a replacement Oregon Health ID card, please call OHP client services at 800-273-0557. You can also visit a local ODHS office and have one printed. To find an ODHS office near you, visit ODHSoffices.oregon.gov.
Some members can switch between a CCO and Open Card at any time. For help switching between CCOs, or to learn more about plan switches, please call OHP client services at 800-273-0557.
To report changes to your contact information or questions about eligibility, applying, or renewing your OHP benefits, please call ONE customer service at 800-699-9075 (relay calls welcome). You can also report changes or renew online at Benefits.Oregon.gov, find a local office at ODHSoffices.oregon.gov, or get help from a local community partner at OregonHealthCare.gov/GetHelp.
The Oregon Health Plan Open Card program will help you with your healthcare needs and support you in having the best health possible. A team of healthcare providers, including a nurse health coach, will work with you to achieve these goals.
It is important for you to know your rights while participating in this program.
You have the right to:
- Get information about the Oregon Health Plan Open Card services and programs.
- Talk to a nurse day or night, about my healthcare.
- Get the names and contact information of my nurse or health coach. I can also ask to talk with their boss.
- Have my information remain private. Oregon Health Plan Open Card will only use my records for providing health services. Oregon Health Plan Open Card can only release my information according to State and Federal laws.
- Be treated with respect as an individual.
- Get these services regardless of my:
- Race
- Ethnicity
- National origin
- Religion
- Gender
- Marital status
- Sexual preference
- Age
- Disability
- Be involved in making decisions about my health. As allowed by law, a family member or guardian may represent me.
- Speak with Oregon Health Plan Open Card staff in my own language. If needed, Oregon Health Plan Open Card will get me a translator for free.
- Choose to not be in the program. If I join, I may quit at any time.
- Be told of the benefits for participating in the Oregon Health Plan Open Card.
- Be educated on how people are chosen for the program.
- Have an honest talk about all health services that might help me and information on available options and alternatives.
- To receive information in a way I can understand.
- Be informed of new health benefits and programs from Oregon Health Plan Open Card.
- Know if this program changes or ends.
- Talk openly about the Oregon Health Plan Open Card Program, and my rights. I also have the right to state my opinion without fear of punishment or discrimination.
- To continue to receive all other DHS and OHP benefits whether or not I participate in this program.
- Exercise all my rights without fear of being treated differently by the Oregon Health Plan Open Card Program.
