Welcome to the Oregon Health Plan Open Card Member Page
OHP Open Card is a type of Oregon Health Plan (OHP) some people get instead of a Coordinated Care Organization (CCO). OHP Open Card helps members who are not in a CCO get their care. OHP Open Card is only available in certain situations.
A CCO is a local network of health care providers working together to care for OHP members in their area. If you are in a CCO, contact them for help accessing care.
Being an Open Card member means:
- You can see any provider who takes OHP Open Card. Call OHP Open Card 800-562-4620 for help finding providers and coordinating your health care.
- Open Card American Indian/Alaska Native members can call CareOregon Tribal Care Coordination at 844-847-9320.
- OHP Client Services at 800-273-0557 can help with billing issues, or questions about Open Card or CCO enrollment status.

We are available from 7:00 a.m. to 7:00 p.m. to answer your calls. We will help you find providers and access services through Open Card. We can help you access resources you may need for health, food, transportation, providers, utilities, housing, and benefit coordination issues.
Care Coordination
Care Coordinators are healthcare professionals who work closely with you to develop a plan to meet all your health and health related social needs. When individuals have complex or ongoing care needs, OHP Open Card can help coordinate your treatment plan with your providers and community agencies
24/7 Nurse Advice
The OHP Open Card Nurse Advice line is a 24/7 medical advice line for OHP Open Card members. If you have a health problem you know isn’t an emergency, you can get advice from a nurse to know if you need to go to the doctor or not. We are available any time of day
