Care Coordination
Case Management
Benefits of our Program
Our teams will help you with accessing resources you may need for health, food, transportation, providers, utilities, housing, and benefit coordination issues. We will help you access services under your OHP plan.
Care Coordination
Care Coordinators are available 8:00 am to 6:00 pm to answer your calls with compassion. Our Care Coordination team is a highly experienced team of healthcare professionals who serve to assist you with your needs.
Case Management
Case Managers are a dedicated team who work closely with you to develop a plan to meet all your health and social needs. When individuals have complex or ongoing care needs, Kepro Case Management can help coordinate your treatment plan with your providers and community agencies.
